Discount Plan Exclusions and Policies:
1. This discount plan cannot be used in conjunction with any insurance plans, discount plans, workers compensation benefits or dental benefits included under an automobile medical insurance claim.
2. This plan can only be used at AV Dental Associates PA, in Jersey City.
3. Plans are non-refundable.
4. Unused benefits cannot be carried over to the following year.
5. Discounts cannot be used in combination with any other special. The discounts are only on the full practice fees.
6. Patient’s portion for any treatment is due before the treatment is provided.
